Enrollment Information for the 2023-24 School Year
- In order to provide a high–quality education to all of our students, Oregon Connections Academy deemed it essential to set an enrollment limit.
- At this time, the enrollment limit has been met, and students who complete all enrollment tasks will be placed on a waiting list and released to enroll based on completion of enrollment tasks.
Grade 12 has closed enrollment for the 2023-2024 school year.

Enrollment for the 2023-24 school year will close on 4/19/2024 at 11:59 PM PT.
- The last day to register: 3/31/2024.

Check Eligibility
You must meet specific state and program requirements to attend Oregon Connections Academy. Student eligibility requirements in Oregon are determined by the Prairie City School District, which is the authorizing body for Oregon Connections Academy. Before enrolling, review the following eligibility requirements carefully to determine whether your student(s) is eligible to participate in grades K–12. Once your enrollment is confirmed, please immediately withdraw your student from their previous publicly-funded school. Students cannot be simultaneously enrolled in two public schools.
Note: Students who are currently expelled from their local school or district may not be eligible to enroll.

Please have the following documents available when you are ready to begin the enrollment process.

A copy of one of the following must be provided for each child in order to verify age eligibility:
- Official birth certificate
- Passport
- Certificate of live birth
- Baptismal papers
- Hospital certificate
- Custody documentation
- Military ID or Driver’s License
- Adoption record
- Green card
To avoid unnecessary delays, provide a copy of one of the items listed above along with your other enrollment documents. If you do not have an acceptable document, please contact the enrollment team at 1-833-591-0251.

As a reminder, the age eligibility is as follows:
- Kindergarten: Any student enrolling in kindergarten must be five (5) years old on or before September 1 in the school year for which you are enrolling.
- First Grade: Any student enrolling in first grade must be six (6) years old on or before September 1 in the school year for which you are enrolling.
- Maximum Age: Any general education student who turns nineteen (19) years old on or before the first day of school will not be eligible to enroll. Any special education student who turns twenty-one (21) years old on or before the first day of school will not be eligible to enroll.

You must reside in the state of Oregon and provide ONE acceptable proof of residency to be eligible for enrollment in Oregon Connections Academy. You can find out what you need below:
Not Sure What is Accepted as Proof?
- Gas, electric, water, sewage, cable, internet, trash, or propane bill from the last 60 days that includes your name and service address
- Mortgage statement from the last 60 days or signed annual lease agreement
- Property tax statement
- Letter from a government agency from the last 60 days
If you do not have any of these documents, please contact the enrollment team at 1-833-591-0251 to discuss acceptable alternatives.
Geographical Limitations
As a reminder, you must reside in the state of Oregon and provide acceptable proof of residency to be eligible to enroll in Oregon Connections Academy. Please note the following important information regarding eligibility:
OAR 581-026-0305 limits enrollment in virtual schools for full-time students to no more than 3% of a school district’s population.

All Oregon Connections Academy students must provide evidence of their immunizations on the Oregon Certificate of Immunization Status Form. This form must be signed by you and, if applicable, a health care provider.
Oregon allows for medical or non-medical exemptions; however, medical exemptions must be verified by a health care provider. Please use the Oregon Certificate of Immunization Status Form if requesting an exemption.

Where Can I Get Immunizations for My Student?
Immunizations can be obtained from doctor’s offices, clinics, or hospitals. In addition, some large chain pharmacies offer immunizations for older children and adults.
In the state of Oregon, most pediatric and family practice clinics offer immunizations. If your child has a regular medical provider, we recommend calling that office for an appointment for immunizations. If your child does not have a regular provider, you can contact your local county health department, or call 1-800-SAFENET (1-800-723-3638). They will be able to help you find a clinic where your child can be immunized.
What if I Need Assistance Paying for Vaccines?
If you need assistance paying for shots/vaccinations, you may be able to get free or reduced immunizations through this program:
- Vaccines for Children (VFC) program is a federally funded program that provides vaccines at no cost to children who might not otherwise be vaccinated because of inability to pay. Children who are eligible for VFC vaccines are entitled to receive pediatric vaccines that are recommended by the Advisory Committee on Immunization Practices.
Immunization Exemptions
A student may be exempt from immunizations for the following reasons: medical exemption or non-medical exemption.
- Medical Exemption: Medical exemptions must be verified by a physician or the county health department.
- Non-medical Exemption: Must be indicated on the Oregon Certificate of Immunization Status. Please sign and date the non-medical exemption section and indicate which vaccinations are exempted. You must also submit a Vaccine Education Certificate of Completion or a Vaccine Education Certificate Health Care Practitioner Document.

Students across Oregon in grades K-12 are eligible to enroll in Oregon Connections Academy. Those who have previously attended public school, private school, or homeschool will be asked to submit academic records for placement.
- Report Card
For students applying for grades K–9: Please submit a copy of the report card from the most recent school year.
The following items should be included on the report card:
- Student Name
- Grade Level
- School Year (example: 2023–24)
- School Name
- Grading Scale (example: 90–100%=A)
- Unofficial Transcript
For students applying for grades 10–12: Submit an unofficial transcript that includes the complete end-of-year grades from the prior school year and any academic documentation (such as a progress report/interim report, class schedule, or marking period/semester report card) issued for the current school year. Additional documentation may be requested for placement.
The following items should be included on the unofficial transcript:
- Student Name
- Grade Level
- School Year (example: 2023–24)
- School Name
- Grading Scale (example: 90–100%=A)
- Credits Earned

- Contact your student's previous school and request the document, providing them with the information outlined above, OR contact your school's district office with the request.
If after attempting these steps you still cannot obtain your student's report card or transcript, please contact the academic placement services team at 1-833-591-0251 to discuss acceptable alternatives.
